- Ration Card.
- Aadhaar Card.
- Bank Details.
- Family Particulars/status and Nomination Details of the Beneficiaries.
- Certificate from the Revenue Department or Social Welfare Department or Anganwadi Officials of the Respective Area.
- For Application: Last Subscription Bill, Board ID Card, Aadhaar Card, Bank Pass Book, Medical Certificate Issued by the Competent Authority (RMO), Accident Report, Discharge Slip.
How to apply

Registration – Step 1:
- The interested construction worker should visit the Office of the Puducherry Building and Other Construction Workers Welfare Board (Industrial Estate, Thattanchavady, Puducherry - 605 009) and obtain the prescribed registration form free of cost.

Fill in all mandatory fields, paste the passport-sized photograph (signed across, if required), attach copies of all mandatory documents (self-attest, if required), and add a declaration stating you are a construction worker, mentioning your worksite.

Submit the duly filled and signed registration form and documents to the concerned authority.

Request a receipt or acknowledgement, ensuring it contains the date and time of submission and a unique identification number (if applicable).

Upon verification by the inspectors under the Act, the worker will be enrolled as a beneficiary with the approval of the Secretary (Board). The beneficiary pays ₹25/- registration at enrollment and ₹100/- per year as an annual subscription; the Board collects ₹300/- as a renewal fee once in three years.

- Obtain the prescribed application form free of cost from the Board office (or print it from the "Application for Welfare Benefits" section, Page No. 77, of the Citizen's Charter), fill in all mandatory fields, attach documents, submit to the concerned authority, and request a receipt. NOTE: The application should be submitted within one year from the date of discharge.
