- The child should be a resident of Madhya Pradesh.
- The applicant should be a child below 5 years of age.
- The child must be severely malnourished or sick and meet at least one of the following criteria:
- Mid-Upper Arm Circumference (MUAC) less than 11.5 cm.
- Weight-for-height Z-score less than -3 SD (standard deviation).
- Bilateral pitting oedema (swelling on both feet).

- No documents are required to avail the benefits of this scheme. The child is identified by health workers (ANM/ASHA/AWW) during community surveys or health check-ups.
